Intermittent Rough Idle

Having some problems with an intermittent rough idle on my 95 Civic LX.
It happens mostly when in gear when stopped at a stop street. It seems to be
worse when backing up in reverse. It comes and goes. Seems to be more frequent lately.
It doesn't do it when cold just after it warms up. Also been noticing what sounds like a clatter when the engine is cold. The car does hesitate sometimes when accelerating but once it gets going it seems to run good. I replaced the fuel filter so that can be ruled out. Any ideas what could be causing this intermittent rough idle, clatter, and hesitation? Thanks for any suggestions!

Put some new plugs in and viola, the rough idle and hesitation went away. The plugs were cheap only $2.49 each so I got lucky. The gap on the old plugs was wider than what it should have been too.
Still has the clatter sound when cold though. It doesn't start right after starting, it seems to start after the engine is running for 15 seconds or so. It sounds kind of like spark knock but not sure if that is what it is. Will maybe have my mechanic listen to it and see what he thinks.
