What is the cause of a blown engine rod?
I understand it is from revving a cold engine - is this correct?

It caused by improper lubrication to the bearing journal of the rod. Can be caused by a weak oil pump to bad clearances in the bearing.

Improper lubrication is one of many reasons for a *blown rod* more common reasons seen at the track and on the street are over revving, inbalance, harmonic vibration, casting flaws, weak rod bolts, core shift in rod casting, seized wrist pin, broken crank...revving a sound motor should not hurt it when cold, not highly reccomended though.
