1991 toyota 4x4 4cyl engine help

I am having bad problems with my starting of my truck, when the engine is cold i have a very hard time to get turned over, then when it is turned over the engine has to have its hard idle for about five minutes before i can drive it, if i try to push the accel. it just trys to die until it get warms, then it can burn the gas,....it like it is flooding... i think it is a automatic choke sensor,,,, dont know where it is or how to check it... when i let off the accel i hear a tick sound from the back of the engine i do not know if thats normal.. i have also had a problem for a long time with when i turn on my ac the engine has a very hard time to idle with it, but does ok, but the problem is the motor will start reving up and down the rest of the day as long as the motor is hot even when the ac is off, but only does this when at idle.. and i replaced the belts to try and help.. no luck... the check engine light came on and then this happened(the starting problem) the engine code is.... blinks twice then palse then two more times... i have replaced all of the fireing system.. rotor cap, button, ngk pluggs, borq warner wires, and has a clean air filter, still has the same problem, has a new fuel filter, had a valve job done 12,ooo miles ago... i am stumpped on this one...  what is my problem.. i want to fix this myself if at all possible..

The fact that the problem goes away when the car is at operating temp. says a lot. because the runs on PROM pre prgammed settings when it's below operating temp. If what you say is correct, you can cancle out any sensors as the problem. Your truck is adjusting for 1 of 3 things  or a combination of the 3: Spark, fuel, or air. It sounds like it's too lean when it's cold (to much air). Try and manually choke the engine when it's cold. Lean the air out by closing down the intake to force a richer mix. See it that makes a difference. Check the fuel pressure and match to spec's.

You also might want to check your timing and vac advance because this will cause it too.

when an engine cranks slowly the most common problems are a weak battery or ignition timing that is advanced too far. The starter could also be defective and not turning the engine over at the speed it should. As far as the erratic idle I would look for a vacuum leak or possibly the EGR valve circuit for a malfuntion or even vacuum hoses that are in the wrong place.
